Pay: $33.82 – $50.73 / hour. Pay is dependent on applicant’s relevant experience

Summary:
UCHealth has an exciting opportunity for a licensed clinician to work inside a program built to decrease use of the 911 system for non-emergencies and increase quality of life. This position focuses on post-crisis intervention and preventative care to individuals out in the community.
In collaboration with Poudre Fire Authority, this clinician will serve individuals that need interventions for a myriad of reasons including but not limited to mental/behavioral health needs, food/housing insecurity, hoarding/unsanitary living conditions, lack of medical care, failure to thrive, and aging in place. These services shall be provided to individuals referred to the program after a pattern of repeated use of Emergency Services.
This role will work with and develop relationships with various referring agencies and provide services to individuals experiencing issues that require screening/assessments, triage, brief treatment, and linking to follow-up service providers.
