Position Summary:
Conduct health education and outreach in Spanish and English to patient groups within a given clinic or community area for assigned patient care. Conduct patient education courses in the area of designated patient care. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include Neonate, Pediatric, Adolescent, Adult and Geriatric age groups.
Detailed responsibilities:
COMMUNICATION – Communicate with qualified clients and invite them to participate in program; interview patients, families, collect and process demographic information; financial aid information and other information as required
SUPPORT SERVICES – Assist patients in locating departments, schedule and coordinate new patient appointments and follow up appointments; refer patient and families to appropriate service and resources
COMMUNICATION – Communicate with appropriate staff regarding any coordination of patient care such as pharmacy and specialty clinics
COORDINATION – Serve as dept liaison with clients and departments and outside agencies; provide quality customer service
EDUCATION – Participate in planning, preparation and facilitation of client education, training and exercises as assigned
TRAINING – Presents training to patient groups in assigned clinics
TRAINING – Provide information, training and support to patients, clients, and families by performing outreach to the community, receiving client referrals, conducting or assisting in educational efforts and responding to requests for support and guidance
OUTREACH – Increase program visibility by performing outreach to the community; participate in community education initiatives; provide collaboration and referrals with community providers to ensure continuity of care
COUNSELING – Assist patients in achieving wellness by providing nutrition counseling, parenting education and support and training in the use of medication and supplies
PRIVACY – Adhere to Hospitals confidentiality and sensitive information procedures
TRAINING – Use training materials, training aids, visual aids and films for training
ADMINISTRATION – Perform various administrative functions such as monitoring expenditures and preparing reports and correspondence; may participate in a variety of research projects to develop service plans; may propose changes to program policies and procedures
DEVELOPMENT – Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops
PATIENT SAFETY 1 – Follow patient safety-related policies, procedures and protocols
PATIENT SAFETY 2 – Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes
PATIENT SAFETY 3 – Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk
PATIENT SAFETY 4 – Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner
PATIENT SAFETY 5 – Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don’t understand or may “not seem right”

Credentials:

CPR for Healthcare/BLS Prov or Prof Rescuers w/in 30 days
Valid New Mexico Driver’s License
UNM Vehicle Operator’s Permit w/in 60 days
Physical Conditions:
Medium Work: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Light Work.
