Practice Details

Conduct pre scheduled in home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members
Visits include:

Comprehensive medication review
Medical history review
Physical evaluation

No prescribing, no ordering labs, no imaging, and no changes to treatment plans
Average visits per day: 6 9
Evaluations are supported by pre populated, patient specific forms
EMR: TBD

Requirements

Board Certification: Required
License: Active Tennessee license required (not willing to license)
Education: Master s degree required
Credentialing Timeframe: 7 days to 2 3 weeks
